Different types of procedures in Bariatric Surgery

Most weight loss surgeries are performed using minimally invasive techniques. Gastric bypass, sleeve gastrectomy, adjustable gastric band, and biliopancreatic diversion with a duodenal switch are the most common procedures.

Gastric Bypass

This procedure is also considered as the ‘gold standard’ of weight loss surgery. It is divided into two components. First, a small stomach pouch, which is created by separating the top of the stomach from the rest of the stomach and the second component is dividing the first portion of the small intestine and the bottom end of the divided small intestine is attached to the newly created small stomach pouch. The procedure is completed after this attachment.

Advantages of Gastric Bypass

  • Generates long-term weight loss.
  • Energy expenditure increases.
  • Reduce appetite and enhance satiety
  • Food consumption
  • Inhibits the amount of food that can be consumed.

Disadvantages of Gastric Bypass

  • More complex operation as a comparison to AGB and LSG.
  • Long term vitamin/ mineral deficiencies.
  • The procedure has a longer hospital stay than the AGB.

Sleeve Gastrectomy

This procedure is performed by removing approximately 80% of the stomach. It works through several mechanisms. First, the new stomach pouch catches a smaller size than the normal stomach and helps to reduce the amount of food that can be consumed. It is as emphatic as the roux-en-Y gastric bypass in terms of weight loss and improvement or remission of diabetes.

Advantages of Sleeve Gastrectomy

  • Restricts the amount of food.
  • Induces rapid and significant weight loss.
  • Short hospital stay in this procedure
  • Prevent hunger, reduce appetite and improve satiety

Disadvantages of Sleeve Gastrectomy

  • It is a non-reversible procedure.
  • Long term vitamin deficiencies.
  • Higher early complication rate than the AGB.

Adjustable Gastric Band

This procedure involves a band that is placed around the upper part of the stomach which is creating a small stomach pouch above the band, and the rest of the stomach below the band. This device works with a smaller stomach pouch, eating just a small amount of food will satisfy hunger. The satisfaction of hunger depends upon the size of the opening between the pouch and the remainder of the stomach created by the gastric band. The shape of the stomach opening can be managed by filling the band with sterile saline.

Advantages of Adjustable Gastric Band

  • Reduces the amount of food.
  • Excess weight loss.
  • No cutting of the stomach.
  • No rerouting of the intestines.
  • It is reversible and adjustable.
  • Lowest for vitamin deficiencies

Disadvantages of Adjustable Gastric Band

  • Slower and less early weight loss procedure.
  • Requires a foreign device to stay in the body.
  • Result in mechanical issues with the band, tube or port.
  • The highest chance of re-operation.

Biliopancreatic Diversion with Duodenal Switch

This is a procedure with two components. First, a smaller, tubular stomach pouch is created by recapture a part of the stomach, very similar to the sleeve gastrectomy. Second, a large part of the small intestine is bypassed. The small intestine carries the enzymes that are essential for the breakdown and absorption of protein elements and fat is reconnected to the last part of the intestine so that they can mix with the food stream. Like other surgeries, BPD/DS initially helps to reduce the amount of food consumption and after some time, patients are able to consume a normal amount of food.

Advantages of Biliopancreatic Diversion

  • Results in greater weight loss than other procedures.
  • Patients can consume normal meals.
  • Decreases the absorption of fat by 70% or more.
  • Reduce appetite and improve satiety.

Disadvantages of Biliopancreatic Diversion

  • Higher complication rates and risks as a comparison to other procedures.
  • Requires a longer hospital stay than the other procedures.
  • Protein deficiencies will be more.
